Left ventricular assist devices and the failing heart: a bridge to recovery, a permanent assist device, or a bridge too far?

چکیده
The field of cardiac mechanical assist devices has achieved a number of striking technical breakthroughs over the past 40 years. Emblematic of the type of important technical accomplishments that have been achieved in this field has been the development of the portable, battery-driven left ventricular assist device (LVAD) for patients with intractable cardiac failure. Although LVADs have been used primarily as a “bridge to transplantation,” a number of centers have now begun to implant LVADs as an alternative to transplantation. Indeed, as the technology in this field improves, it is entirely conceivable that LVADs will evolve into small, unobtrusive devices that will run on small, portable, long-lasting battery supplies that will not require external connection to the outside. This, in turn, will allow LVADs to serve as a very reliable alternative to transplantation for many patients with advanced heart failure who cannot receive transplants or who cannot be weaned from LVAD support.

LVAD is a mechanical pump supporting a weak heart function and blood flow. Sometimes, the heart may not recover fast enough to take over the pumping action immediately after surgery, in such patients a temporary support device has been employed to maintain the pumping action until the patient’s own heart recovers. This device can be considered as a temporary alternative before the process of ar...

Background: It has not been yet developed a ost suitable method to treat right ventricular failure due to pulmonar artery hypertention resulted from ventricular arrhythia. Though some case reports about patients ready to heart transplant using left assist device or biventral assist device as bridge have been published in Journals, but nocase concerning application of centrifugal pump for treatm...

The implantation of a mechanical circulatory device for end-stage ventricular failure is a possible therapeutic approach in adult and pediatric cardiac surgery and cardiology. The aim of this article is to present mechanical circulatory assist devices used in infants and children with special emphasis on extracorporeal membrane oxygenation, Berlin Heart assist device, centrifugal pump and Medos...
متن کاملCirculatory assist devices 2000: an update.
Mechanical circulatory assist devices have been used in clinical practice as a bridge to transplantation since the late 1960s. In 1982, the first totally artificial heart designated as permanent therapy was implanted, but relatively few totally artificial hearts are implanted today. In the last several years, there has been a shift toward the use of left ventricular assist devices as a bridge t...
متن کاملInspiratory muscle training in a patient with left ventricular assist device.
The use of mechanical assist devices as a 'bridge' to cardiac transplantation, as a "bridge" to recovery, or as a destination therapy in end-stage heart failure, is increasing. Studies of different modes of rehabilitation in this population are lacking. We report the benefits of inspiratory muscle training on the exercise capacity and quality of life in the recipient of a left ventricular assis...
